Your First Visit
At SmileHaven Dental Center, we strive to provide you with the best possible experience from your first visit through to any treatments that might follow. We have provided an outline of what you can expect as well as any items we may need from you so you can be prepared for your first visit.
Patient Forms
Here you will find a list of our patient forms as well as post surgery care instructions. All new patients are required to fill out our Patient History Form. You can complete this form online in the comfort of your own home. If you prefer to fill out forms in the office, please allow yourself the time to do so before your appt.
Dental Wellness Club
We have created a Dental Wellness Club to help our patients achieve optimal dental health. We understand that many people are not able to keep regular “preventative” maintenance, and then when a problem occurs, it is often more serious, and the cost is more than they can easily afford.
